You are kindly requested to send documents by fax (+88-02-887-1120), e-mail, or mail proving that you are visually handicapped in order to be our membership. In case of sending them by fax, mail, or e-mail, take notes saying subscription to Music Rehabilitation Center, and be sure to write your name and name and phone number. [ Chapter 1 General Principles ] Article 1 (Objective) The objective of this agreement is to describe basis matters regarding use or procedure for members for Music Rehabilitation Center website provided by Music Rehabilitation Center for the Visually Handicapped annexed to Siloam Visually Handicapped Welfare Center (Hereinafter "Music Rehabilitation Center"). Article 2 (Definition of terms) Paragraph 1. The terms used in this agreement shall be as follows: 1. "A member" means a person who enters into a membership agreement with Music Rehabilitation Center and has an ID therewith to have services from the website. 2. "ID" means is combination of letters and figures selected by an individual and given by Music Rehabilitation Center for identification of each member. 3. "Password" means combination of letters and figures selected by an individual to ensure whether it belongs to each member and protect rights of each member. 4. "Membership agreement" means an agreement between Music Rehabilitation Center and a member in order to receive services from the website. 5. "Cancellation" means that either Music Rehabilitation Center or a member cancels the membership agreement. Paragraph 2. The terms used in this agreement shall be defined and interpreted pursuant to governing acts and practices if described otherwise in Paragraph 1. Article 3 (Effect and modification of agreement) Paragraph 1. This agreement shall take effect by means of notifying members through on-line on website or by other means. Paragraph 2. Music Rehabilitation Center might be able to modify this agreement for rational reasons, and in case of modification of agreement, it shall be notified without delay pursuant to the method referred to in Paragraph 1. However, such modification regarding member's right or obligation shall be notified at least 7 days in advance. Paragraph 3. In case where a member disagrees with modified agreement, he or she can stop using website service at any time and cancel the membership agreement. A member who continues to use website service even after effective modification date of agreement is deemed to agree with modification of agreement. Article 4 (Application of agreement) This agreement shall apply to services from the website, but any matters that are not described in this agreement shall apply pursuant to the provision of the Telecommunications Basic Act, The Telecommunications Business Act, The Information & Telecommunication Network Use Promotion, etc Act and other governing Acts. [ Chapter 2 Membership ] Article 5 (Entering into membership agreement and its unit) Paragraph 1. Membership agreement can be established with an approval of Music Rehabilitation Center regarding member's application for membership. Paragraph 2. Membership agreement shall be created by user ID unit, each one person having only one ID instead of two or more IDs. Article 6 (Subscription to use) Paragraph 1. In a case where a user want to subscribe to service, he or she logs onto the website in a real name and subscribes to membership, while Music Rehabilitation Center shall notify contents of this agreement to an applicant for subscription to membership. Paragraph 2. All the information filled in the subscription form by members shall be deemed as real data, therefore no false information can be protected under the law, further service availability can be restricted to those members. Paragraph 3. Members are classified into the Visually handicapped member and no visually handicapped member, and the Visually handicapped member, holding welfare cards, should send documents proving identification of the Visually handicapped by fax (02-887-1120) or e-mail (braillemusic@hotmail.com). No visually handicapped member can make use of any service bulletin board, member board except for the Visually handicapped-exclusive service. In case where he or she waits without getting certified after applying for subscription to membership as the Visually handicapped, only the service available to no visually handicapped member is provided. Paragraph 4. Members can receive information after log-in about general on-going membership state such as notification of receipt after applying for subscription, subscription verification state, etc. Article 7 (Approval of subscription to membership) Music Rehabilitation Center might be able to approve an applicant for subscription to membership after confirm identity certification procedure, while notifying each member of entire service information. Article 8 (Disapproval of or restriction to approval of subscription to use) Paragraph 1. Music Rehabilitation Center shall not be able to approve subscription to membership falling under any of the following categories in cases of: 1. Applying on behalf of others or in a fake or false name 2. Filling in subscription to use with a false information 3. Making it difficult to approve subscription to use for the reasons ascribed to customers Paragraph 2. Music Rehabilitation Center might withhold approval of subscription to membership till the cause of service availability system defection is removed, if any. Paragraph 3. Music Rehabilitation Center shall inform applicant for subscription to membership of the fact that subscription to use is disapproved or restricted pursuant to Paragraph 1 and Paragraph 2 if that happens. Article 9 (Allotting and changing ID, etc) Paragraph 1. Music Rehabilitation Center allots ID to members pursuant to Music Rehabilitation Center's notification, service information guidance. Paragraph 2. ID can be changed in discussion with user if falling under any of the following categories: 1. In cases where the user uses ID the same as his or her telephone number or personal ID number which can damage privacy 2. In cases where ID can give rise to aversion or traditional custom 3. In cases for any other rational reasons Paragraph 3. Any matters relating management and changing ID shall be determined pursuant to this agreement and Music Rehabilitation Center's notification, service information guidance. [ Chapter 3 Contractual Party's Obligations and Responsibilities ] Article 10 (Music Rehabilitation Center's obligation) Paragraph 1. Music Rehabilitation Center shall promptly handle opinions or complaints issued by a member in the course of using website service if such is deemed reasonable. However, if it is deemed difficult to handle without day, its cause and handling schedule shall be notified to the member by e-mail or telephone. Paragraph 2. Music Rehabilitation Center shall make efforts to provide members with stable and convenient service. Paragraph 3. Music Rehabilitation Center shall repair and restore the website service in case of sudden system disability for the purpose of supplying a continuous and stable service. However, the website service might be stopped or pause in cases of natural disaster, emergency or other unavoidable occasions. Article 11 (Protection and management of individual information) Paragraph 1. Music Rehabilitation Center shall protect member's individual information created in the course of services from the website as well as member's individual information provided from application for subscription to membership pursuant to the provisions of the Information & Telecommunication Network Use Promotion, etc Act, or other governing acts. However, Music Rehabilitation Center's individual information handing policy shall not apply to linked sites other than Music Rehabilitation Center's official site. However, individual information provided from application for subscription to membership can be used in the following items. 1. Fulfilling membership agreement such as service provision, etc 2. Membership management 3. Transmission or notification to members Paragraph 2. Music Rehabilitation Center shall not provided individual information to the third party without a member's consent. It shall, however, be otherwise in case of the following occasions. 1. In cases where it is requested for criminal investigation, requested by KOCSC, or requested pursuant to provisions or procedures under other governing Acts 2. In cases where it is deemed necessary to statistics record, academic research or market research, but provided only in the form of format whose individual identify cannot be identified 3. In cases where a minimum information (name, address, telephone number) is given to delivery company Paragraph 3. Hyomyung Music Team Leader (Taehun Oh) is a person who is in charge managing entire website member's individual information, whose address and contact number is 1717, Nambusunhwan-ro, Gwanak-gu, Seoul, 02-880-0670 respectively. The person in charge of managing each individual information might be changed later and it will be notified on website if ever. Paragraph 4. Members can read his own individual information at any time, correcting errors himself. Reading and modification might be done in the same way as that of subscription to membership. For further information on its method, see the provisions under notification, information guidance. Paragraph 5. Members might be able to cancel the website membership agreement at any time, in the meantime Music Rehabilitation Center should delete cancelled member's individual information at the same time when its cancellation is approved. Article 12 (Member's obligation) Paragraph 1. Members shall notify, without delay, Music Rehabilitation Center of any modification of matters described in the membership agreement such as address and contact number if any. However, real name and e-mail cannot be modified for the service management. Paragraph 2. "Members" shall modify on line, send by e-mail, or notify Music Rehabilitation Center of any modification of matters written on application for membership, if any. Paragraph 3. Any disadvantages occurs as a result of failure to notify Music Rehabilitation Center of changes pursuant to Paragraph 2 shall not be in the responsibility of Music Rehabilitation Center. Paragraph 4. Members shall not be involved with any of the following deeds: 1. To illegally use other member's ID 2. To disturb stable operation of service by sending large amount of information 3. To continuously transmit advertising information (spam mail), pornography, violent message as opposed to recipient's intention 4. To cause malfunction of information and telecommunication equipment or destruction of information 5. To infringe intellectual property such as copyright belonging to a third party or Music Rehabilitation Center 6. To defame or disturb work performance of Music Rehabilitation Center and other third party 7. To perform illegal election campaign referred to in authentic interpretation of National Election Commission 8. To harm good customs or other social order 9. To make commercial availability of information from the service without Music Rehabilitation Center's consent 10. To violate the provisions under 'information and telecommunication related acts' 11. To change information posted by Music Rehabilitation Center, and/or transmit or post information (computer programs, etc) other than the information set by Music Rehabilitation Center 12. or to carry out other illegal or irrational deeds Paragraph 5. Members shall pay attention to ID and password management and take responsibility for any matters as a result of lending his or her own ID to others. Paragraph 6. Members shall observe this agreement and governing acts, without giving rise to serious damage or disturbance to work performance of Music Rehabilitation Center. [ Chapter 4 Restriction to Service Use or its Suspension ] Article 13 (Restriction to service use or its suspension) Paragraph 1. Music Rehabilitation Center might be able to restrict or suspend use of whole or part of service if is deemed unavoidable for the reasons such as warfare, war, natural disaster or other similar national emergency, or suspension of telecommunication service pursuant to 'The Telecommunications Business Act'. Paragraph 2. Music Rehabilitation Center shall inform members of its cause and period of restriction when it restricts or suspends use of whole or part of service pursuant to Paragraph 1. Article 14 (Restriction to service availability) Paragraph 1. Music Rehabilitation Center, if a member falls under any of the following categories, might be able to restrict or cancel to use member's service. However, in a case of restricting to use or cancel the service, its intention shall be notified to members at least 7 days in advance to receive any refutation from members. 1. In cases of failure to perform obligations under Paragraph 2, Paragraph 3, Paragraph 4 of Article 12 2. In cases where false record is revealed 3. In cases of falling under objects for restriction to use by KOCSC Paragraph 2. Members taken measures not to use membership pursuant to previous paragraphs might claim refutation, and Music Rehabilitation Center might delay suspension of service availability till its confirmation. Paragraph 3. Music Rehabilitation Center shall cancel the measures against canceling service availability if it proves that the cause of suspension of use is removed during the period. Paragraph 4. Music Rehabilitation Center shall abide by the provisions under Music Rehabilitation Center's notification, service information guidance when it comes to restricting member's service use. [ Chapter 5 Pause and Termination ] Article 15 (Pause) On occasions where a member would like to pause service availability for certain reasons or extend the service term, he or she send the cause and service term to Music Rehabilitation Center. Article 16 (Termination) Paragraph 1. In a case where a member would like to cancel membership agreement, he or she personally visit, call, or apply for it on website bulletin board. Paragraph 2. Music Rehabilitation Center terminate service after the following day if the application for cancellation is received pursuant to previous paragraph. Paragraph 3. Music Rehabilitation Center might be able to apply for membership again with a lapse of one week in the case of such member who terminates service. However, in case where a member applies for membership again whose service availability is terminated pursuant to Paragraph 1, Article 14, its approval shall be determined by a website staff meeting. [ Chapter 6 Supplementary Provisions ] Article 17 (Material and contents management) Paragraph 1. Music Rehabilitation Center might be able to take necessary measures such as deletion, relocation, or cancelling registration, etc to material falling under any of the following categories to effectively operate and establish a stable communication culture. 1. Contents and material deemed to infringe other's legal benefit, requested by relevant parties for its deletion, or material and contents potentially causing Music Rehabilitation Center to be indicted or sued therewidth 2. Material and contents containing virus deemed to do harm to service 3. Material and contents deemed to cause harm to other member's service availability 4. Material and contents requested by the national agency for its deletion pursuant to information & telecommunication related telecommunication and criminal related acts Paragraph 2. Music Rehabilitation Center shall notify members of the fact in advance in case where an action is taken pursuant to categories falling under above. However, it shall be notified after action is taken in case of emergency. Article 18 (Copyright and restriction to use) Paragraph 1. In cases where members provide material and contents while posting contents and transmitting material without having explicit expression as opposed to, it shall be deemed that Music Rehabilitation Center has a complete right to inform communication to the Public. Paragraph 2. Copyright or intellectual property posted by Music Rehabilitation Center shall belong to Music Rehabilitation Center itself. Paragraph 3. No part of information or files from the website may be reproduced, transmitted, published, or distributed in any form or by any means. Paragraph 4. The deeds falling under above paragraph might be withdrawn pursuant to notification, service information guidance determined by Music Rehabilitation Center. [ Supplementary Clause ] 1. (Enforcement Date) This agreement shall take effect as of January 1, 2012.

Music Rehabilitation Center processes personal information for the following purpose: 1. The processed personal information are not used for any other purposes except for the following. If they are to be changed, we plan to ask for prior consent. A. Member registration and administration We process your personal information to confirm your intention to join, verify and confirm your identity according to the membership service supply, maintain and administer your membership right, verify identity according to Limited Identity Verification System, prevent from taking advantage of the service illegally, confirm the legal representative's agreement when gathering personal information of children below the age of 14, notify any information, deal with any problems, and to preserve any record for grievance meditation. B. Civil Petitions Treatment We process your personal information to verify identity of the civil petitioner, confirm the civil affair manners, contact and notify for fact-finding, and to notice for the result. C. Supply of goods or service We process your personal information to offer service, contents, adjusted service, and to verify identity and age. 2. Processed personal information items A. Internet personal information Nationality, e-mail, first name, last name, password, posession of disability, birthdate, phone number, address and zip code of the place of delivery, belonged group, approval for e-mail reception, and more. 3. Period of time taken to process and retain personal information The Music Rehabilitation Center processes and retains personal information within the period of time based on the term about it by the law or within the time agreed when gathering personal information from the principal agent. A. The period of time to process and retain personal information in every aspect is: 1) Member registration and administration: until withdrawal But, in the following case, until it finishes. A) When going through investigation for violating relational statute, until the investigation is over. 2) Civil Petitions Treatment: until 3 years after the end of handling civil affairs 3) Retainment of personal verification information according to the law regarding the promotion of information and communication network use and protection of information enforcement ordinance section 29: 6 months after the end of posting the information in the bulletin board. 4. The fact that one has the right to refuse to agree and its disadvantages The user has the right to refuse to agree on gathering personal information by the Music Rehabilitation Center. When carried it out, member registration will not be proceeded but will be able to take advantage of the services(ex. new announcements) offered to non-members.
